Admissions

To apply for Admission to Mingo Valley Christian School, please return the Admissions Application completed and signed (one per student), along with the following forms for each student:

• Copy of recent report card

• Copy of recent standardized test scores

• Copy of immunization records (complete)

• Immunizations Required

• Admissions Questionnaire

• Pastor Recommendation Form (Pastor may send this separately)

• Authorization for release of records

• Parent Authorizations for Enrollment

• Computer and Internet Acceptable Use Policy and Agreement

• Tuition Payment Preference - Financial Policy

When all forms are received, and if space is available, a family interview and entrance test will be scheduled.
Entrance test fees: K4/K5 $10.00; 1 st -12 th Grade  $20.00
